LONG TERM ACUTE CARE (LTAC) >>

 

PATIENT CARE COORDINATOR
NURSING ADMINISTRATION

Evening/Night/Weekends

Graduate of an accredited School of Nursing, BSN preferred. Current DC RN license or eligible. Current CPR/ACLS certification. Two years nursing work experience in an acute care setting, previous nursing supervisory experience preferred.


 

REGISTERED NURSE
2-EAST/2-WEST/2-WEST RESP/HI-OBSERVATION

Full–Time, Part-Time & PRN
Days/Evenings/Nights with weekend rotation

Graduate of an accredited school of nursing. BSN Degree preferred. Current DC RN license or eligible. Current CPR certification. Recent nursing experience, preferably in a long-term acute care setting.


 

WOUND CARE NURSE
NURSING ADMINISTRATION

Full–Time/Days

Graduate of an accredited school of nursing. Preferred candidates will have two years in nursing with prior experience in wound care and currently licensed in DC. Current CPR certification.

 


 

PATIENT CARE TECHNICIAN
2-WEST

Evening and Night Shift with Weekend Rotation

High school diploma or equivalent. Nursing assistant training and current certification, or EMT training and current certification, advanced training in an LPN or RN program with at least one year of program completion. Six months previous nursing assistant or patient care technician experience in an acute care or long-term care environment.


 

CASE MANAGER
CARE MANAGEMENT

Full-time, Days

Graduate of an accredited school of nursing or LPN program. BSN Degree preferred. Current DC RN/LPN license or eligible. 2-4 years recent medical/surgical experience. Familiar with standard case management concepts, practices, and procedures and area hospitals and long-term care facilities. Will act as a liaison between referring healthcare facilities and HMH personnel to ensure necessary services are available and care is provided promptly and effectively. Must have own transportation. Certain degree of creativity, latitude and flexibility is required.
